Pharmacogenetic Testing

This test analyzes how your genes may affect your outcomes with medications commonly prescribed to treat depression, anxiety, ADHD, and other mental health conditions.  The test provides your clinician with information about which medications may require dose adjustments, may less likely to work for you or may have an increased risk of side effects based on your genetic makeup.

Payments

It is your responsibility to know and to pay your co-pay, co-insurance, or deductible at the time of your visit. Your insurance company will still be billed to validate your copay or deductible. Any amount not covered by your insurance will be charged to your credit card on file.

Additional Fees

  • There is a $45.00 fee for checks with insufficient funds.

  • If you need to be billed, there is a $10.00 billing fee per appointment, per billing cycle.

  • Balances over 30 days may incur a 3.5% monthly finance charge.

  • Balances past due for 90 days may be sent to an outside collections’ agency.

Prior Approval (if required)

from your insurance company must be completed before your visit. Please call your insurance company to help determine your coverage as well as your deductible.

Copays

are sometimes a fixed amount, or they may also be a co-insurance which is a percentage of what is billed. Please be sure to verify your patient responsibility with your insurance to avoid any surprises.

Mind Works Team accepts:

  • VISA

  • Mastercard

  • Discover

  • HSA cards

A valid credit/debit card is required to be on file when setting up an appointment time and will be charged in the event of a no show for the full amount of the visit which is non-refundable. Additionally, your credit card/debit card will be charged for any remaining balance due for co-pays and deductible requirements in addition to any fees not covered by your insurance.

Policies of Services

Prescriptions

will be written with enough refills to last until your next appointment. We recommend scheduling your next appointment at the end of each session so that we can be sure you are following your treatment plan and do not run out of medication.

Additionally

MWW Team is not a Forensic Psychiatry office. Forensic, court work and forensic psychiatry, is a specialty that we are not trained in but are happy to refer you to someone who does this type of therapy.

Medication Refills

are done during the appointment time only. If something unexpected occurs, please have your pharmacy fax a refill request. Do not email or call about refills. Please allow up to five business days for the refill to be approved, if appropriate, and schedule an appointment as soon as possible.

Mind Work Wellness Team does not provide

court evaluations or court testimonies. Please inform us immediately if you are involved in, or plan to go to court. We accept clients only under the specific agreement that they will not involve Felicia McNeil, PMHNP-BC or MWW Team in any legal matters, including but not limited to: child custody, workers’ compensation claims, and criminal cases. We do not provide legal opinions, perform psychiatric evaluations for legal purposes, nor do we testify for disability or child custody cases.

Controlled Substances

such as benzodiazepines and stimulants are tightly regulated by the DEA. These medications can be very useful when indicated but require more monitoring and must be part of a comprehensive treatment plan. If you do not adhere to the agreed upon treatment plan, then we will be unable to continue to provide care to you. There will be no early refills of controlled substances for any reason.

Disclaimer

If we are ordered to testify in proceedings, it seriously undermines our therapeutic practitioner-patient relationship, is very disruptive to the office routine, and is unfair to our other patients.

Letters and Forms

If you are requesting a letter to be written on your behalf or forms to be completed, please keep in mind that your request will take 3+ business days to complete. Our medical provider must approve and sign all letters before they can be picked up/mailed/faxed. The minimum turn around time for letters is one week, so please request them 1-2 weeks in advance.  Fees for providing written documentation, filling out forms, letters, etc. start at $25 and increase due to the complexity of the communication.

Forms start at $25 and increase

Disability Claims

If you are requesting assistance in completion of a disability claim, please make note of the following:

****Short Term Disability and FMLA Paperwork ONLY! We do not process paperwork or evaluate for Social Security Disability!****

1. MWW Team does not "give" disability. We can only report symptoms and response to treatment to the company that handles your disability insurance.

2. We do not backdate disability.

3. If regularly scheduled appointments are not kept, we will notify your disability carrier.

4. Our providers first priorities each day are to see scheduled patients; therefore, they will complete letters and forms as time permits. Disability paperwork requests take 7 to 10 business days to complete.
