We are now pleased to offer Pharmacogenetic Testing! 

This test analyzes how your genes may affect your outcomes with medications commonly prescribed to treat depression, anxiety, ADHD, and other mental health conditions.  The test provides your clinician with information about which medications may require dose adjustments, may less likely to work for you or may have an increased risk of side effects based on your genetic makeup.

Ask about our medication-assisted treatment (MAT) services.
MAT is the use of medications in combination with counseling and behavioral therapies for the treatment of substance use disorders. Our clinic currently uses Suboxone.

Payment Information 

Prior Approval

If required, prior approval from your insurance company must be completed before your visit. Please call your insurance company to help determine your coverage, as well as your deductible.

Copays

Copays are sometimes a fixed amount, or they may also be a co-insurance which is a percentage of what is billed. Please be sure to verify your patient responsibility with your insurance to avoid any surprises.

A valid credit/debit card is required to be on file when setting up an appointment time and will be charged in the event of a no show for the full amount of the visit which is non-refundable. Additionally, your credit card/debit card will be charged for any remaining balance due for co-pays and deductible requirements in addition to any fees not covered by your insurance. 

It is your responsibility to know and to pay your co-pay, co-insurance, or deductible at the time of your visit. Your insurance company will still be billed to validate your copay or deductible. Any amount not covered by your insurance will be charged to your credit card on file. 

Additional Fees

  • There is a $45 fee for checks with insufficient funds.  

  • If you need to be billed, there is a $10 billing fee per appointment, per billing cycle. 

  • Balances over 30 days may incur a 3.5% monthly finance charge. 

  • Balances past due for 90 days may be sent to an outside collections’ agency.

Prescriptions

Patients' prescriptions will be written with enough refills to last until the next appointment.
We recommend scheduling your next appointment at the end of each session so that we can be sure you are following your treatment plan and do not run out of medication.

Medication Refills

Medication refills are done during the appointment time only.
If something unexpected occurs, please have your pharmacy fax a refill request. Do not email or call about refills. Please allow up to five business days for the refill to be approved, if appropriate, and schedule an appointment as soon as possible.  

Prescription Drugs

Controlled Substances

Controlled substances such as benzodiazepines and stimulants are tightly regulated by the DEA. These medications can be very useful when indicated but require more monitoring and must be part of a comprehensive treatment plan.
If you do not adhere to the agreed upon treatment plan, then we will be unable to continue to provide care to you. There will be no early refills of controlled substances for any reason.
